 If you have been diagnosed with PD and would like to participate in our research:

 What we need from you:

1. A clinical history with a definite diagnosis of PD.
2. A signed form consenting to the research (we will supply this).
3. A blood sample – this can be arranged to be couriered to our lab from anywhere in South Africa.
4. Information on who else has PD in your family
